Antegrade visualisation of the coronary sinus for left ventricular pacing.

چکیده
INTRODUCTION Failure to achieve left ventricular pacing remains one of the obstacles to cardiac resynchronisation therapy. METHODS A new technique for the placement of left ventricular pacing leads in the tributaries of the coronary sinus is described. Antegrade visualisation of the coronary sinus is accomplished by selective coronary angiography and a hydrophilic 0.032" wire is advanced along the coronary sinus. This facilitates advancement of a coronary sinus sheath over a multipurpose diagnostic catheter. A hydrophilic angioplasty wire is then utilised for negotiation of the inferior left ventricular vein and successful placement of the left ventricular pacing lead. RESULTS In 5 patients in whom cannulation attempts were unsuccessful despite prolonged fluoroscopy time (39.85 +/- 9.6 min), the new technique resulted in successful placement of the left ventricular pacing lead within an average fluoroscopy time of 6.5 +/- 2.1 min. CONCLUSION Antegrade visualisation of the coronary sinus via selective coronary angiography and the use of a hydrophilic angiography wire may facilitate successful implantation of a left ventricular pacing lead in difficult cases.

Antegrade and Retrograde Decremental Conduction Properties of an Accessory Pathway Associated with the Coronary Sinus Musculature
A 32-year-old man underwent catheter ablation of an orthodromic atrioventricular reentrant tachycardia. The sinus rhythm electrocardiogram exhibited a normal PQ interval and no delta waves, but atrial pacing produced a prolonged PQ interval and wide QRS morphology with right bundle-branch block due to antegrade accessory pathway (AP) conduction. During the tachycardia, atrial double potentials ...

Electrophysiological and epicardial mapping studies are described in a patient without pre-excitation who had intractable recurrent paroxysmal supraventricular tachycardia. Electrophysiological studies revealed fixed VA conduction times during both rapid ventricular pacing and coupled ventricular stimulation. Catheter mapping of atrial activation during retrograde conduction and during induced ...

The effects of altering the site of electrical activation on responses to isoproterenol (ISO) and treadmill exercise were examined in mongrel dogs instrumented for long-term measurement of left ventricular pressure, left ventricular dP/dt, coronary blood flow, cardiac output, left ventricular diameters, and mean arterial pressure and O2 content in the coronary sinus and aorta. During spontaneou...
متن کاملCoronary sinus mapping to differentiate left versus right ventricular outflow tract tachycardias.
Outflow tract ventricular tachycardia (OT-VT) can originate from several different segments of the outflow tract. Various ECG criteria have been proposed for localization of OT-VTs. We present two patients, one with left and one with right OT-VT. We used local ventricular electrograms in the coronary sinus to localize the focus of the OT-VT. Mapping of local ventricular electrograms in the coro...
متن کاملThe Effect of Steroid Elution on the Performance of Coronary Sinus Leads for Left Ventricular Pacing in Dogs
Consistently low pacing thresholds of steroid-eluting leads are well documented for conventional lead positions [1-10], but not much comparative information is available on the extent and duration of the steroid effect in coronary sinus leads for left ventricular (LV) pacing. In this study, we compare the electrical performance of the coronary sinus leads Corox OTW steroid vs. Corox OTW (both B...
